The Health Action Planning Group aims to do the following things:

• Let group members meet each other a share ideas. 
• Look at how health action planning and health facilitation is going within the services that are payed for by Adult Social Care for people with a learning disability (auditing). 
• Develop services and resources to help make sure health action planning and health facilitation happens.
• Report to the Leicester City Learning Disability Partnership Board. 
• To share with others information about the work the group is doing.

The Health Action Planning Group has a Framework this says what we are trying to do. The Framework was agreed by the Partnership Board in 2004.  To look at the framework click here: Health Action Planning Framework

The Action Plans tell you what we are trying to do.  It follows on from the Framework.

This Action Plan is from 2005 – 2008, it was written and agreed by the Partnership Board in 2005.
